Why we ask this
A change on the board — appointment, resignation, or both — must be reported to MCA on Form DIR-12 within 30 days of the board or shareholder decision. Beyond that window, additional fees kick in at ₹100 per day, capped at 12× the normal fee, and the registrar can refuse other filings until DIR-12 is regularised. The CIN, the nature of the action, and the incoming or outgoing director's details are what DIR-12 itself asks for, so those are the fields here.
